What's PrEP?

PrEP are medications you can take preventively to prevent HIV infection. 'PrEP' is an abbreviation; full name is Pre-Exposition Prophylaxis. The tablets contain a combination of two HIV-inhibiting substances, which together ensure that HIV cannot take hold of your body when you contract the virus. Thus, you prevent the HIV infection from progressing and you do not become seropositive.

PrEP is a fantastic invention. By taking medication temporarily at a time in your life when you are at increased risk for HIV, you can avoid having to take medication perhaps for life. Because if you have HIV you have to continue to take medication daily, and these are heavier HIV inhibitors than with PrEP.

Since 2018, PrEP has been allowed to be prescribed in the Netherlands. Thanks in part to PrEP, we have seen HIV rates continue to drop in recent years, which is of course incredibly good news. The goal now is to make PrEP easily accessible to all people at increased risk of HIV. The GGD has a program where those at the very highest risk can go for free. Unfortunately, there are long waiting lists for that in many places. Any doctor who is well-versed in the national guideline is allowed to prescribe PrEP, including all general practitioners. However, not every general practitioner is willing to do so. You cannot get PrEP without a prescription.

How does PrEP use work?

There are two ways to use PrEP.

The first way is daily, you then take 1 tablet every day. With this, you know you are protected at all times.

The second way is intermittent, where you only use tablets around sex. Here you do need to know approximately when you are going to have sex so you can take tablets in time. You then take 2 tablets at least 2 hours before sex and should continue with 1 tablet a day for the next 2 days. Read more about the usage schedule at mantotman.co.uk and watch the video.

*Anonymously make your PrEP appointments and do the tests anonymously with us! At which pharmacy you submit your prescription afterwards you can choose yourself. There is also an online pharmacy: PrEP-direct. Staying completely anonymous is unfortunately not possible with PrEP as with other medications, because the prescription must contain your personal information.

For whom is PrEP suitable?

PrEP is intended for people who are at increased risk of contracting HIV during sex. We know of the following groups that this applies to them, especially if they do not use a condom:

  • men who have sex with men
  • transgender persons who have sex with men
  • sex workers
  • people with a partner from the above groups

There are also other conceivable situations in which people are at increased risk for HIV. Are you unsure if you are at increased risk for HIV? Do you doubt whether PrEP is something for you? You can always visit us for an informal intake, then we will see if PrEP is suitable for you.

Are you on a course of PEP (post-exposure prophylaxis) and think you will be at risk more often in the future? Then you can often continue with PrEP immediately afterwards. Check with the GGD that accompanies you with the PEP about the possibilities.

How safe is PrEP?

How effective is PrEP?

When used properly, PrEP is incredibly effective. It works at least as well as a condom in stopping HIV. When PrEP users do contract HIV it is usually because they forgot to take tablets and sometimes because the virus was resistant to the substances in PrEP. Only 1 case has been reported worldwide of HIV infection under proper PrEP use where the virus was not resistant. So that is extremely few, as it has been used in America since 2012 and in Europe since 2016. So you can trust PrEP if you take it correctly.

Other STDs are not stopped by PrEP. Since it has been used, we have seen an increase in gonorrhea and syphilis infections in the Netherlands. So it is best to use a condom as well. By using PrEP and condoms together, you are best protected. For many people, this helps to have fine, carefree sex.

What side effects does PrEP cause?

It has now been conclusively proven that PrEP is safe to use. Moreover, PrEP is well tolerated by most people. We regularly see mild side effects in the beginning. These usually go away by themselves after a few days to weeks of use. Do you experience them a lot or persistently? Report it to us, then we will think with you about solutions.

Side effects at first:

  • headache
  • gastrointestinal complaints
  • lethargy, feeling weak
  • vivid dreams, insomnia
  • skin rash

There are also some side effects that occur only very occasionally, in less than 1 in 100 PrEP users, but can be more serious. For this reason, we keep a close eye on everyone who receives PrEP through us with regular check-ups so that we can intervene in time. The side effects are reversible and not permanent.

Rare side effects:

  • impaired renal function
  • osteoporosis (which does not lead to more bone fractures)

Is PrEP compatible with other medications?

HIV-inhibiting medications cannot always be combined with other medications; sometimes this causes interactions. We naturally check before you start PrEP whether this applies to your home medication. Also, always let us know if you use self-care medication or supplements from the drugstore. Next, what is incredibly important during PrEP use is that you always mention that you are taking PrEP when another doctor wants to prescribe you new medication. This way they can check if the combination is safe! If you have any questions, you or your doctor can always contact us.

In case you are now wondering, "So can I combine PrEP with drink and drugs? The answer is: yes, fortunately there are no known interactions between alcohol, the well-known party drugs and PrEP.

How does PrEP care through OneDayClinic work?

Making an appointment is very easy. Often you can get an appointment the same day. You book it online or by calling us.

Step 1. The intake

Your initial consultation with our doctor/nurse will take 30 minutes. In the appointment confirmation email, you will be sent a document with helpful information about PrEP. Please read this carefully before the appointment! During the appointment we will give you additional information about PrEP and take a questionnaire to see if PrEP is right for you. After this, you can choose to have all necessary tests taken immediately.

We will then do a large package of STD tests and check your kidney function. We will also talk about hepatitis A&B vaccination.

Step 2. Request your recipe

You will have the results within 3 working days. As soon as they are known you can email us to request your prescription. One of our doctors will look at your results and, if everything looks good, make a digital prescription for the first 30 tablets within a few days. You can then submit this prescription to the pharmacy of your choice or upload it on prep-direct.com.

Step 3. Come on a check-up

Everyone using PrEP should basically come for check-ups every 3 months. This way we can keep an eye on whether everything is going well and take regular STD tests.

There is also a mid-term check-up, 1 month after you start the PrEP pills. You won't get a new prescription until the results are in each case. Has an STD been found? Then you usually get the new prescription at your treatment appointment.

Please note, if you use PrEP daily, it is helpful if you come for a check-up 1 week before your tablets run out! That way you can get your tablets back in time and won't run out.

What does PrEP cost?

The biggest cost of PrEP use is doing the STD tests every 3 months. At the GGD, these tests are free. At the GP you pay for them from your deductible through your health insurance, so you have a maximum amount per year. At OneDayClinic, you pay for them yourself, outside of your health insurance.

The first appointment with us costs 50 euros if you do not take tests and 270 euros if you do take tests.

The second appointment (after 1 month) costs 90 euros.

The follow-up appointments usually cost 230 euros and sometimes 270 euros.

You must always pay for the medication yourself; it is not reimbursed.

25,- euros per 30 tablets is a common price, but it can be cheaper. In addition, some pharmacies charge much higher prices, sometimes as much as 60,- euros. It is a good idea to call your pharmacy first to ask what it costs there.

Where can I buy PrEP?

You can buy PrEP as soon as you have a prescription, you do this at a pharmacy. You can do this at your local pharmacy or online. It is worth asking first if your pharmacy has it in stock, because otherwise the delivery time is and what it costs there. If you prefer to order online, you can use prep-direct.co.uk. There you can upload our digital prescription, pay via a Tikkie and have the tablets shipped to your home address.

How can I safely stop taking PrEP?

If you want to stop taking PrEP, there are a few things to consider.

First, it is important that there is enough PrEP left in your body long enough to prevent HIV infection.

  • Have you had sex in your anus? Then continue PrEP for at least 2 more days.
  • Have you had sex in your vagin* or neovagin*? Then continue PrEP for at least another 7 days.

In addition, it is important to have another HIV test 3 months after you last took PrEP.

Have you done these things? Then you have safely stopped.

With a referral from your family doctor, we can create an invoice that you can submit to your health insurance company. Depending on your health insurance company, this invoice can be approved or rejected. If the health insurance company approves it, the costs will first be deducted from your deductible and when that is exhausted, you will get your money back. A godsend, because life is expensive enough as it is! Everyone should be able to enjoy safe, carefree sex, whether they have more or less to spend, we think. PEP and PrEP

There is only one letter difference between the terms, but the real difference between PrEP and PEP is a lot bigger. PrEP has the "r" of "pre," indicating that you have to take it beforehand. PrEP only works if it is already present in your blood before you become at risk. PEP has no 'r' because the first letter comes from 'post'. This indicates that you take it afterwards. PEP is a heavy course of virus inhibitors that you can take shortly after you've been at risk (within 3 days, but the sooner the better). With this, you still have a chance that the infection can be fought off and you never become seropositive. Read more about the differences on our page about HIV. What is PrEP treatment?

This is an understandable question, as medications are usually taken as treatment. Yet you can't really call PrEP a treatment, because it is preventive. You take these pills precisely to prevent you from getting sick. If you take PrEP properly, you can prevent yourself from getting HIV. So that way, you don't end up needing lifelong HIV treatment, which is the goal. Do you currently have an increased risk of getting HIV? Then it's a good idea to consider taking PrEP medication for a while during this period of your life, so you know you won't soon be stuck with pills for the rest of your life.

What is a PrEP cure?

This is another understandable question, but because PrEP is not a treatment but preventive medication we prefer not to call it a cure. A cure implies that you take medication for a while and then you are cured, but when you use PrEP you are actually not sick. You can only use PrEP if you don't already have HIV. This is because if you have HIV then you need full-blown HIV treatment and PrEP is not enough for that. Do you think you have an increased risk of HIV? Then it's worth looking into PrEP.
